Re-homing Agreement

• I agree that the dog is being re-homed for myself and will not be sold or given to another party.

• I agree that the dog will not be allowed outdoors without proper supervision. When taking my dog outdoors, they will be on a secured harness.

• I agree that this dog is to be a companion animal, not a guard dog. The dog will live inside my home, not outdoors.

• I agree to care for the dog in a humane manner and be a responsible animal guardian. This includes giving it adequate food, water, shelter, attention, and medical care.

• I agree that if at any point I can not keep the dogl, I will return it to the center without requesting a fee.

• I understand and agree that the center makes no guarantees about the dog’s temperament and is not responsible for future damages or injuries caused by it.

• I give the center permission to call my home at any reasonable time to assure that the dog is being properly treated and cared for.

• I agree to keep the center informed of my current home address and phone number.
